Please use this page to register with one of our TRANSFORM cohorts but only if you have already been invited to do so by your cohort leader.

By registering you will gain access to the online content of the TRANSFORM programme including video teaching sessions, reflections, and assignments.

If you already have an account on the website:

Please click here to login first. After logging in the website will redirect you back to this page and you will see the form below populated with your details.
Select your cohort from the "membership" list
Select your hub from the "Hub" list
Scroll down, confirm your details and click "submit"

If you do not have an account on the website already:

Select your Cohort and Hub from the list below
Add your email address, choose a username and create a password which will set you up with a new account.
Scroll down to fill in the other details on the form
Click submit
